Why would anyone get their fillers dissolved? They’re the number two aesthetic treatment worldwide! Millions of people are queuing up for fuller cheeks, contoured features and plump, shapely lips.
However, some celebrities are rocking the boat by deciding to get their filler removed, encouraging us to re-evaluate our attitude towards this procedure.
I’ve never felt comfortable with very large lips, but thanks to celebrity role models and social media stars, overfilled lips are becoming the norm. I’m glad to see people challenging the trend, with Love Island star Charlotte Crosby making the headlines by having her lip filler chemically dissolved. She put a question mark over the value of excessive lip fillers, while cosmetic doctors are exploring the risks of overfilling.
For example, Dr Harris deliberately draws attention to the distortion caused by overfilled lips and cheeks. He believes that too much filler leads to ‘alienisation’, with features enlarged outside the normal range for the individual in question.
However, it’s not always an unscrupulous doctor or an excessive treatment that leads to these strangely exaggerated results. Many people forget what they looked like before treatment and return for more or the substance takes longer to dissolve than we originally thought. The patients believe that they’re in need of a top-up while it’s still in their system, leading to a double dose of filler.
We all think about our appearance. Low confidence is often the motivation to have an aesthetic treatment such as dermal fillers, as it helps us to embrace how we look. However, cosmetic treatment has been linked to an extreme preoccupation with appearance, known as body dysmorphic disorder (BDD). People with BDD spend a disproportionate amount of time obsessing over minor or imagined flaws. This can be enormously upsetting, so it’s no surprise they turn to cosmetics to try and fix the problem.
It’s a difficult issue for us practitioners to identify, especially as fewer than 10% of patients with BDD are satisfied with the results of their treatment. They move on to other practitioners to correct this perceived problem or their anxieties transfer to another area of the face or body. Their constant quest to fix the flaw leads to distorted, overfilled features.
Now, not everyone with overfilled features is suffering from BDD. After all, we all worry about our appearance to some extent and BDD is the extreme end of the spectrum. In my practice, I tend to see patients who are delighted with a filler treatment, but quickly forget what they looked like before the procedure. They want a more discernible difference, so they come back wanting another treatment.
It’s easy to see how people’s faces become overfilled, especially as fillers may last much longer than we think. Alice Hart Davis, a beauty journalist and author, discovered that 35ml of dermal filler is still present in her face, even though we expect it to have dissolved naturally in less than two years.
Not at all! They help millions of people to look youthful, including me! However, we do need to take care.
In my clinic, I limit the amount I’m prepared to place at one time, assessing my patients via ultrasound to determine if any filler remains. This helps me decide if it’s suitable for them to have more treatment or whether I should direct them to an alternative, of which we have many. When I do conduct treatments, I tend to use Juvederm, which creates wonderfully subtle and natural-looking results.

The excitement of Christmas has faded away, replaced by a new year! We often see this as a new start and a chance to reset, as we abstain from bad foods, alcohol and an unhealthy lifestyle. We start a diet instead or take up running. However, you want to be careful with this ‘new year new me’ philosophy!
There’s one big problem with this mindset – it doesn’t work! Studies show that 23% of people give up on their New Year’s Resolutions after one week, while 43% expect to give up before the end of January. In fact, less than 10% of people successfully continue with their new lifestyle!
‘New year new me’ won’t motivate you for very long, especially after a slip-up. You need a plan for a long-term goal because you can rarely make lasting changes based on an impulse. As a dentist, I believe in making small changes, which culminate in significant improvements.
Take alcohol for example. From a skin perspective, we’re in favour of reducing your intake. Too much alcohol dehydrates the skin and reduces elasticity, leading to fine lines, creases and a dull, dry complexion. It can also give rise to conditions like psoriasis and rosacea, to say nothing of the increased risk of skin infections. On the other hand, going cold turkey can be a mistake. It doesn’t allow any flexibility for what can be a big lifestyle change (depending on how much you’re used to drinking). Similarly, you might be invited to a social event and feel like you’re unable to enjoy a drink, leading to regret, resentment and an eventual return to bad habits.
Instead of being stern with yourself, follow a little-by-little approach. Try giving up alcohol during the week and allowing yourself 3-4 units at the weekend or at a party. It may feel like a little change, but imagine the impact after one, two or three months! You’ll be in a much healthier state, without the inevitable disappointment caused by the ‘new year new me’ attitude.
There’s more to January than giving up bad habits. Many of you contact me in the winter, asking me to restore your faces after the festive period. Your skin cells are practically self-cleaning, so they don't need harsh ingredients or double-cleansing. This is because every 21 days or so, your cells renew themselves, which means cleaning your skin within an inch of its life won’t do it any favours! Cleansing too frequently will disrupt the renewal process and break the skin’s natural defences, allowing moisture to escape and pollutants and bacteria to get in, which isn’t exactly healthy and won’t result in glowing skin!
When you’re stressed, your body releases a hormone called cortisol. Cortisol is good for you if you're experiencing a ONE-OFF stress event, like being chased by a lion! However, persistent, chronic stress and the resultant high cortisol levels can wreak havoc on ALL the organs of your body… the biggest being the skin.
We can’t eliminate stress in our lives, but we can learn strategies to deal with it. I find that practising good sleep hygiene, cutting back on my alcohol consumption, meditating and doing some restorative exercises like Hatha Yoga works for me and leads to glowing skin. I also encourage myself to be kind - to myself and others, even if that might mean saying ‘no’ to someone. You might not feel good doing this, but your stress levels will benefit from it and so will your skin. The skin is a complex organ and so are the processes that keep the cells working well. I won’t go into biochemistry and bore you with science, but without the right nutrients, your skin cells simply won’t be able to do their job properly.
Avoiding processed foods and eating your leafy greens will go a long way. However, our modern lifestyles don’t make this easy. Avoid overusing products or using inappropriate ones on your face. Overuse is more common than you think, especially products that are heavy and don’t allow your skin to breathe. This can cause acne, perioral dermatitis and clogged pores - which results in small white bumps called milia.
Using inappropriate impulse buys can also be harmful to your skin. These products may contain chemicals and essential oils of unknown origin. When it comes to Botox, particularly Botox for frown lines, we must consider the chicken and the egg question. What does Botox have to do with chickens and eggs, you may ask? Well, it’s to do with the old question of which came first. The chicken or the egg?
You would think that your happiness is down to your mindset but it just so happens that scientific research points towards the opposite being true. Botox for frown lines has mood-boosting abilities and ridding ourselves of negative expressions, like scowling, can make us happier.
Coles recently proved this by combining data from 138 studies and over 11,000 participants. The results were exciting as they show that smiling more does actually make you happier, but also that scowling and frowning more will make you feel sadder.
Whatever your thoughts on this study are, it’s my opinion that they show how the mind and body interact to shape our feelings. Unsurprisingly, many factors have been shown to influence our emotions, so you may not be able to smile your way to happiness. However, I’ve witnessed something in my patients who get regular Botox treatments - they smile more and frown less!
Many of my patients have Botox for frown lines. As a result of treating this area, they aren’t able to frown fully, but that’s not to say that they can’t still move their muscles. A ‘frozen’ look would be quite strange - but they certainly can’t scowl anymore. Apart from boosting their confidence levels, this also seems to make them happier!
I did some digging and it seems that there is proof that Botox treatments may have other mood-altering effects. In particular, reducing anxiety levels and depression. There are several theories on why this happens and while none of them are proven, what is certain is that patients in these studies benefited from Botox for frown lines.
It seems that my patients are right - their treatments aren’t just for aesthetic purposes, but they also help to lift their mood and give them confidence in the things they do that matter so much to them. Socialising, going for a job interview or even a date night feels different when your mood is better. It’s simply sunshine in a vial!

Fashions come, fashions go. I was a teenaged Goth in the 1980s and my hair was so stiff with hairspray that I should have been deemed a fire hazard. Every generation has their own style and we’ve since moved on from mullets and mohawks, with modern-day young people after big and bold in a different sense. Lip enhancements are in fashion and good lip fillers are essential in their quest for the perfect pout.
The hunt for attractive, full-bodied lips is strewn with pitfalls. Women are seeking bigger lips, but not grossly inflated pouts. Unfortunately, celebrities and influencers have normalised overfilled and misshapen lips and Generation Z have seized upon that look without question. Of course, it’s up to each generation to choose their own beauty norms, but lip fillers were never meant to be used this way! This misrepresentation of a great treatment means that you might not even know that you can achieve a natural-looking pout with good lip fillers.
Lip fillers are made from a gel-like substance called hyaluronic acid (HA). This forms naturally in our bodies, cushioning our cells, hydrating the skin and providing additional structure for our organs. The HA in our bodies is identical to that which is used in lip fillers. The only difference is that in fillers HA is stabilised, so that when it’s inserted into the chosen area it can do the job it’s meant to do, enhancing and beautifying by absorbing massive amounts of water.
It’s crucial that lip enhancements are carried out by expert practitioners to minimise complications and get the very best, natural-looking results.
I have been treating patients for years using HA fillers and have been trained in all the necessary skills. It can take years to learn how to place the filler so that it supports the lips and fills lip lines while remaining invisible to the naked eye, which means that good lip fillers should be the reserve of qualified and experienced experts.
A practitioner also needs a comprehensive knowledge of different lip filler products, so that they can choose the best one for a particular patient’s needs. Having the right concentration of HA is of primary importance, as it will affect the softness and smoothness of the result as well as the integration of HA into the tissues. The technology used to stabilise the HA will also impact the outcome.
Essentially, good lip fillers cannot be found on the cheap at non-specialist parlours. Comprehensive research, approval by the Food and Drug Administration (FDA) and safe delivery costs hike the prices up, meaning that quality stabilised products all come with a price tag. A bargain basement lip filler treatment probably won’t use premium products and is unlikely to give you the natural voluptuous lips you are looking for. When searching for good lip fillers, an expert is the only way.

Fat loss treatments are incredibly popular, especially for pockets of fat which just won’t shift, no matter how well we eat or what exercise regimes we follow. However, stories of negative experiences with specific procedures might cause us to doubt their safety. I think it would be a shame to give up on the benefits of all fat loss treatments, so let’s investigate.
80s supermodel, Linda Evangilista, would strongly discourage fat freezing as a form of fat loss treatment. She recently came forward to share her story. Linda claims that her CoolSculpting® treatment left her disfigured when she suffered from a side effect that increased, rather than decreased, fat in the affected areas. The complication, medically termed paradoxical adipose hyperplasia (PAH, not to be confused with pulmonary arterial hypertension), meant that she managed to gain rather than lose fat in critical areas, despite dieting and exercising.
Such complications are rare, but it caused her great distress. No matter how infrequently a problem occurs, I believe the experience of the individual matters greatly.
PAH only occurs in 0.0051% of patients, suggesting that by and large what Linda went through is not a likely outcome for this fat loss treatment, although no unifying risk has been identified with this FDA-cleared treatment.

The question of whether or not fillers for wrinkles are safe has been one that people have asked since dermal fillers first became a well-known feature in the world of aesthetics. I have no doubt that it ranks very highly on Google searches too!
When it comes to answering this question in the simplest terms, the answer is a resounding yes! There’s a lot of evidence to show that dermal fillers for wrinkles (and several other purposes) are a safe and effective treatment option. However, there’s much more to this question than you might think at first glance. Even the safest treatment can become problematic in the wrong hands.
It is my belief that the wrong practitioner performing any kind of injectable treatment, such as dermal fillers for wrinkles, cannot be trusted. When it comes to choosing a clinic and practitioner you can trust, you may have to do some snooping. Based on a clinic’s Instagram or website, you should be able to quickly ascertain what qualifications they hold. Sadly, when it comes to some places, even Sherlock Holmes would be at a loss. However, if you can find what you’re looking for, you should soon be able to look a practitioner up on a professional database. Anyone with qualifications, whether they’re a doctor, nurse or dentist, should be on a professional register, as this holds them accountable for the treatment they provide.
The next step is to dig a little deeper. In order for someone to be able to provide fillers for wrinkles safely, they should have a Level 7 qualification, or an equivalent Master’s degree. This level of practical training ensures that an individual has the knowledge they need in order to perform dermal fillers for wrinkles safely. These qualifications take years to obtain, but they provide practitioners with invaluable knowledge.
The last thing to check is the premises in which you’ll be treated. Anyone who claims to be a practitioner, but continues to operate from a location that is primarily meant for another purpose is unlikely to be legitimate. In my professional opinion, these places will not meet your treatment needs. Aesthetic procedures, whether we’re discussing fillers for wrinkles or chemical face peels, need to be performed in safe and clean conditions. Many locations simply don’t meet the standards necessary, which other practices pride themselves on.

Ageing doesn’t just occur on the surface of the skin. It happens to the rest of the body too, right down to our bones. If you have found yourself wondering how to prevent to prevent ageing, frantically searching to little avail, there are some things that can help. The trouble is, most of us are so focused on our fine lines and wrinkles that we forget to consider other changes that might be taking place. This is why my multi-layer approach is so important. There are things a magnifying mirror can’t show you, and taking a close-up look at your skin can really make you feel rotten. Fine lines and wrinkles will always look worse when magnified, worse than they are in fact. The truth is, the odd line is often unnoticeable and can add significant character to the face.
I believe that a few lines here or there can look very natural, especially for people over a certain age. Of course, when too many appear, it can make a person feel unhappy with their appearance. This is where wrinkle-relaxing treatments that use Botulinum toxin can help you. These treatments work to prevent the skin muscles from creasing and forming wrinkles. This is most effective on the upper third of the face, but what’s happening below the surface? That’s what the multi-layer approach seeks to address.
It may shock you to know, but your facial bones shrink considerably with age and they lose density too! In fact, it’s said that the ‘bone sets the tone’. In other words, if there isn’t enough support from the bones in your face, gravity will cause your skin to sag, making you look much older than you are. These include dermal fillers and thread lifts. Such treatments offer support and structure, allowing you to achieve a youthful look without surgery.
When answering the question, "how to prevent ageing?", every single layer of the skin matters. That’s why specialists, like myself, recommend working on refining skin texture and building collagen in the superficial layers of the skin to see the best results. I find that if skin health is addressed prior to a procedure, whether through targeted skincare or a collagen boosting treatment, then other treatment results are more impressive too. If a person’s skin isn’t healthy to start with, they could be line-free, with no noticeable sagging, but they might still have a dull, lacklustre complexion.
That’s why I follow the multi-layer approach. I like to focus on improving skin health, minimising fine lines by relaxing muscles, and replacing lost support with dermal fillers, threads, or both. I find that patients who are willing to try this approach maintain a youthful appearance as the years go by. Many of my patients have been with me for several years and would say they look much younger than they did at the start of their treatment journey.

Did you know that the Botulinum toxin molecule (BoNT) is capable of much more than wrinkle reduction? Botulinum toxin injections are often referred to as Botox®, which is actually a registered trademark of Allergan Inc., a global pharmaceutical company.
Botulinum toxin itself is a purified substance derived from bacteria that has been used for decades to block nerve signals to the affected muscles in which it is injected. The result? Softened lines and wrinkles, and much happier individuals. More recently, Botulinum toxin has also been proven to help those who suffer with hyperhidrosis too. But it doesn’t end there...
Over the years, we have been so much in awe of its muscle-relaxing properties that we seem to have ignored many of Botulinum toxin’s other useful applications. Botulinum toxin is one of the most significant advances in dermatology the world has ever seen, and believe it or not, it was simply a happy accident.
Jean Carruthers, a relatively unknown ophthalmologist from Vancouver, Canada, was injecting her patients with BoNT to control their eye spasms, but when several of these people reported a reduction in their frown lines, she started to suspect she had discovered something revolutionary. The effects were so dramatic that she decided to treat herself, and she was blown away by the results!
Needless to say, back in 1987, Jean had no idea that her accidental discovery would result in millions of people feeling happier and more confident, even in 2022! Women in particular have benefited from Botox® treatments over the years, mainly due to the fact that their faces age differently to men’s. Female faces lose volume and structure much quicker, often making their faces appear sullen or angry, which means Botox® treatments are more effective over time. Many people don’t know that BoNT can attach itself to almost any cell in your body and change how that cell works. Yes, really! As a result, it can do much more than just relax the muscles. Those in the aesthetics industry don’t simply tell their customers this to entice them, it has been scientifically proven to be the case. This is incredibly important because what happens at a cellular level allows us to understand the other uses of the BoNT molecule.
Do you want to keep your skin looking plump and youthful? Then Botulinum toxin might just be what you need. The BoNT molecule has been found to prevent collagen from breaking down, but the best news is that it actually helps your body to produce more. It’s something I see a lot in my practice, the patients who have Botox® on a regular basis have a glow about them. I call it ‘the BoNT glow’, now we know why!
There’s much more to this magical molecule too. BoNT has even proven to be effective when used for non-surgical facelifts in very diluted concentrations. Low levels of Botulinum toxin can be used at several different points across the entire face, causing the skin to shrink and tighten. Although we have been aware of this effect for a while, it has only been confirmed recently that Botulinum toxin can be used in this way to deliver spectacular results.
Two other unexpected conditions that can be treated with Botulinum toxin are acne and rosacea. In those with acne, the BoNT molecule is able to attach itself to the sebaceous glands, rendering them sluggish and unable to produce as much sebum as they normally do. This leads to smaller pores and a significant reduction in acne. We’re not yet sure as to how or why the BoNT molecule is able to treat rosacea, but it reduces the severity of facial flushing, which is welcome news for rosacea sufferers who may not have responded well to other treatment methods.
